• The insured shall immediately notify the loss or damage to the dealer / retail outlet of the lens.
• If the loss is due to burglary, theft, immediate complaint should be made to the police.
• If the lens is broken or damaged, the same is to be deposited with the dealer / Retail outlet against acknowledgement.
• Depreciation rate @8.33% per month is applicable from the date of purchase.
• The following documents are to be submitted to the dealer / Retail outlet against the acknowledgement
  1. Certificate of Insurance duly stamped by the retail outlet
  2. Claim form
  3. Original Invoice
  4. Authenticity card
  5. Broken lens ( In case of breakage / accident)
  6. Police record (In case of theft or burglary)
  7. Maximum limit is Rs.25,000/- only

• Loss or damages to frames.
• Scratches, denting, chipping and cracking including at the points of fitting
• Theft from any unsecured vehicle
• Wear and tear
• Misplacement or mysterious disappearance
• Willful act or negligence of the purchaser
